According to the Alabama Department of Public Health, Chapter 420-3-23, Body Art Practice and Facilities, your permanent records must include the following:

Client information

Record the client’s full legal name, date of birth, and residential address to verify age and identity.

Document the specific date of the procedure; for multi-session tattoos, record every individual session date.

Include a clear description of the design and its exact location on the client’s body.

Log the full name of the tattoo artist who performed the procedure to link the work to the licensed operator.

The client must sign the record to verify the information provided and consent to the procedure.

For minors, obtain written consent from a parent or guardian in person, and retain a photocopy of their government-issued ID.

Clients must sign a written consent form acknowledging risks like permanence, infection, and potential allergic reactions.

A medical history form covering conditions like diabetes, allergies, and heart disease must be signed by both the client and the operator.

Provide written care instructions to every client, including the facility’s name, address, phone number, and advice to consult a physician if infected.

The operator must verify the client is not under the influence of alcohol or drugs; a client attestation supports this assessment.

Retain records permanently. Keep the last three years onsite; older records may be stored offsite within the county if retrievable upon request.
