Tattoo waiver requirements in Alaska
According to the Alaska Administrative Code, 18 AAC 23.315, tattoo shops must maintain client case history cards and related operational records containing the following information:
Client information
Record the client’s full name, residential address, and phone number.
Procedure details
Document the date of the tattoo procedure, the type of procedure performed, and the location of the procedure on the client’s body.
Artist information
Include the name of the practitioner who performed the tattoo procedure.
Operational records
Maintain an inventory of instruments, sharps, and tattoo inks, including manufacturer names and lot numbers when applicable. Shops must also retain written sanitary procedures, monthly spore destruction test records, and manufacturer instructions for equipment used in the shop.
Recordkeeping
Keep each client case history card on the shop premises for at least three years after the client’s most recent procedure and make the records available to the department when required.
