Tattoo waiver requirements in Florida

According to the Florida Department of Health (Chapter 64E-28, F.A.C.), your waivers must include the following:

Artist name

The name of the tattoo artist.

The customer’s name, age, and birthdate.

A description of the tattoo and its location on the customer’s body.

The customer’s signature.

The artist’s signature.

The date, or dates, the tattooing procedure was performed.

No tattooing under 16. For ages 16–17, verify minor and parent or guardian photo IDs, collect any required proof of age, and obtain notarized parental consent on the state form.

Aftercare information must be provided to the customer verbally and in writing.

Keep customer records and minor consent forms for at least two years, and have current licensing-period records available onsite for inspection.
