According to the Iowa Administrative Code 481—492.9(5) governing the practice of tattooing, tattoo establishments must maintain permanent client records that include the following information:

Record the client’s full legal name and date of birth. A copy of a valid government issued photo identification must be obtained and retained in the client record.

Document the date the tattoo procedure was performed.

Include the name of the tattoo artist who performed the procedure.

The client must sign the record confirming the tattoo procedure.

Printed aftercare instructions must be provided to the client after the tattoo procedure is completed.

Client records must be kept confidential and retained for at least three years. These records must also be made available to the Department or the designated inspection agency if requested during an inspection or investigation.
