Studio details
Record the name of the studio and its certificate number on every client form.
Client information

Include a description of the tattoo, its specific location on the body, and the type of procedure performed.
Tattoo artist
Record the name of the registered tattoo artist who performed the service.
Risk risclosure
Provide written information stating that tattooing poses a risk of infection, is permanent, and that removal may leave scars.
Aftercare instructions
Provide written, verbal, or electronic instructions that cover site care, restrictions, possible side effects, signs of infection (such as fever or excessive swelling), and a directive to consult a physician if infection symptoms occur.
Client attestations
The client must sign a statement attesting that they are not pregnant, not under the influence of drugs or alcohol, and have not ingested anticoagulants (blood thinners) within the past 24 hours.
Record retention
Store all client records and consent forms for a minimum of two years. These documents must be made readily available to health inspectors upon request.
