According to the Minnesota Department of Health, Statutes §146B.07, your waiver records must include the following:
Client information
The client’s full legal name, date of birth, current address, verification using a government-issued ID, the client’s signature confirming the information is accurate, and the date signed.
Health disclosure
A completed health questionnaire documenting relevant medical conditions that may affect the procedure or healing, including diabetes, bleeding disorders, skin diseases or sensitivities, seizure or fainting conditions, use of blood thinners, and any other relevant conditions.
Authorization to decline service
A statement acknowledging the technician may decline service if a disclosed condition creates a health or safety risk.
Informed consent
A signed consent confirming the client understands the procedure, including acknowledgment that tattoos are permanent and removal may cause scarring.
Procedure details
The procedure date, a description or design of the tattoo, the name of the tattoo artist or technician, and the technician’s license number.
Aftercare instructions
A statement confirming the client received verbal and written aftercare instructions, understands normal healing versus signs of infection, and knows when to seek medical attention, with client signature or initials acknowledging receipt.
Record retention
All client records must be retained for at least three years and made available for inspection to the department of health when requested.
