According to the Nebraska Department of Health and Human Services, 172 NAC 44-012.03, tattoo facilities must maintain client records containing the following:
Client information
Record the client’s name, address, and date of birth.
ID verification
Keep a copy of the client’s federal or state identification showing a current photograph and date of birth.
Health disclosures
Document any known client allergies and any condition that may affect or hamper healing.
Procedure details
Record the tattoo artist’s name, procedure date, location on the client’s body, and a description of the tattoo procedure performed.
Clients under 18
Keep a copy of the parent or legal guardian’s photo ID, a consent form signed by both the parent or guardian and the client, and documentation verifying the parental or guardianship relationship.
Record retention
Keep records confidential and available to the Department upon request. Adult records must be retained for 3 years, records for clients under 18 for 5 years, and the previous year of client records must remain on-site.
