Tattoo waiver requirements in North Carolina
According to the North Carolina Department of Health and Human Services, Division of Public Health, Tattoo Rule 15A NCAC 18A .3206, your retrievable waiver records must include the following:
Client information for each tattoo
The client must record or verify their name, address, phone number, and date of birth, and provide their signature.
Procedure date
The date the tattoo procedure was performed.
Record retention
Waiver records must be kept for at least two years.
