Tattoo waiver requirements in Ohio
According to the Ohio Administrative Code, Chapter 3701-9-04, every tattoo shop must keep these client records for each procedure:
Client Info
The client’s full name and address.
Tattoo Date
The date of the procedure.
Tattoo Colors
The colors and manufacturer of all inks, dyes, or pigments used.
Tattoo Placement
The body part placement of the tattoo.
Record Keeping
Client procedure records must be kept for at least two years.
