Tattoo waiver requirements in Oklahoma
According to the Oklahoma Administrative Code (OAC 310:233-3-6), your permanent client records must include the following:

Record the client’s full legal name, date of birth, and residential address.
Proof of identity
A photocopy of a valid government-issued photo ID must be kept on file.
Procedure information
Document the date the tattoo procedure was performed.
Tattoo details
Record the identification and exact body location of the tattoo performed.
Artist information
Include the tattoo artist’s name and license number.
Medical disclosure
Clients must be asked to disclose conditions that may affect healing, including diabetes, bleeding disorders, skin conditions, allergies, pregnancy or nursing, medications, and other relevant medical history. Clients can choose not to provide this information by providing a signed statement indicating their refusal.
Risk disclosure
Clients must be informed of potential risks, side effects, and complications associated with tattooing, even when sanitation standards are followed.
Aftercare instructions
Written tattoo aftercare instructions must be provided, including care steps, infection warning signs, and when to seek medical attention. These instructions must include the tattoo establishment’s name, address, and phone number.
Complaint information
Clients must be informed how to file a complaint with the Oklahoma State Department of Health.
Record retention
Client records must remain confidential, be available to the Department upon request, retained for 3 years, and properly destroyed afterward.
