Customer name

The client’s full name.

Date and time of the tattoo.

Identification or description of the tattoo.

Operator or tattoo artist name.

Records must be retained for at least two years and made available to the Department upon request.

Provide verbal and written aftercare before the procedure. The written sheet must include infection guidance, tell the customer to see a physician at the first sign of infection, and list the establishment’s name, address, and phone number, and it must be available to the Department on request.

No tattooing under 16 unless ordered by a physician. Under 18 requires an in-person parent or legal guardian with written consent, and the operator must verify ID and guardianship.

No tattooing anyone who appears intoxicated or under the influence of alcohol or drugs.
